What Is Fractional Laser Skin Resurfacing?

The invention of Fractional Laser Skin Resurfacing was the last true revolution in clinical skin rejuvenation and dermatological scar reduction.

“Developed in 2004 by Solta Inc. in the USA, Fraxel™ was the first Fractional Laser Platform in the world.” Dr Ehsan Jadoon

Fractional laser devices deliver very narrow (microscopic) columns of laser energy into the skin to remove old and damaged skin cells, leaving the surrounding cells intact. This stimulates the skin’s natural healing process, and the old cells are replaced by fresh, healthy and glowing new skin cells.

The ability to deliver laser energy into the skin in this “fractional form” offers several critical benefits over traditional full-field laser resurfacing.

  1. There is far less discomfort and downtime compared to full-field ablative laser.
  2. There is minimal risk of skin burns.
  3. More energy can be delivered into the skin safely and effectively to stimulate more collagen and tissue remodelling.
  4. The fractionated laser energy can penetrate much deeper into the skin and therefore is able to break deeper scar tissue. Something, the full-field laser is unable to do without burning the skin.

Most cosmetic clinics use the word “Fraxel” to describe any fractionated laser treatment they perform regardless of the device or technology they use.

What are the key indications of Fractional CO2 Laser?

Fractional CO2 Laser has two main clinical indications:

Skin Rejuvenation (skin resurfacing)
Scar Reduction (Acne scars, stretch marks, other types of scars)

How is the treatment performed?

The treatment area is first numbed using a specially formulated, high-strength topical anaesthetic cream. Once the skin is numb, the treatment is performed using the treatment parameters which are based upon the patient’s skin type, treatment requirement and downtime concerns.

Is the treatment painful?

We are asked this question every time. Most patients who have searched for Fractional CO2 Laser treatment online have seen some ” patient journey” videos from the US which look quite gruesome!

We have no idea why any doctor would use such high settings to cause such significant bleeding and pain when treating an average person for facial skin rejuvenation.

The laser settings used by Dr Jadoon at Medaesthetics are carefully selected to minimize any discomfort or downtime associated with the treatment while delivering noticeable clinical results.

Most of our patients tolerate laser treatments extremely well.

How do I prepare for the treatment?

Preparing the skin for a minimum of two weeks prior to CO2 laser resurfacing is critical in achieving hassle-free results. (Four weeks for patients with darker skins). This involves:

Avoiding direct sunlight (UV exposure)
Wearing protective clothing, hats, sun-glasses and sun-blocks.
Daily Skin exfoliation.
Application of topical Vitamin A, B and C (serums or creams).
Prophylactic topical hydroquinone (for patients with darker skin types who are at risk of developing PIH).
Prophylactic anti-viral tablets (for patients with a history of cold sores).
At Medaesthetics, each patient is provided with the complete skin preparation kit and instructions before the treatment is booked.

What is the recovery like?

As mentioned above, the recovery from this procedure depends upon the treatment-intensity.

Light-Medium Intensity treatment:

Light-medium intensity settings are primarily used for skin rejuvenation treatments. The downtime is about five to seven days of mild swelling, redness, bronzing and occasional peeling. There is very little discomfort associated.

Medium-High Intensity treatment:

The treatment settings are used for treating scars and stretch. The downtime is about seven to fourteen days of redness, swelling, oozing, bronzing and occasional peeling. There is some discomfort associated with the treatment which usually lasts for first two-three days only.

What are the potential risks and complications associated with CO2 laser skin treatment?

CO2 Lasers have a well-recognized and a very high safety profile when performed by an experienced operator. Human skin can react adversely to anything that may cause irritation or evoke an allergic response. It doesn’t even have to be a laser.

Adverse reactions associated with any Laser treatment are rare and of transient nature. These include:

Prolonged redness of the treated area (usually lasts for a few weeks).
Temporary swelling (due to the heat which causes fluid buildup).
Dermatitis like skin response.
Cold sores reactivation.
Pigmentary changes (may last for several weeks)
Careful patient selection, thorough skin-preparation and post-laser care are critical to minimize any adverse effects.

What is the post-treatment skincare after the CO2 Laser treatment?

Patients have to:

Strictly avoid direct sunlight until the skin has fully healed.
Do not wet the treated area for the first 48 hours.
Keep the area constantly covered with post-treatment ointment/cream provided to you by us.
Do not apply any products or make-up over the treated area which may irritate the healing skin.
Strictly avoid disrupting or picking any scabs. This can result in scarring and prolonged pigmentary changes. Let the scabs fall off naturally.
